No you don’t.

Thanks for confirmation; you get it with Nissan's to encourage you
to not go to an independent garage.
With Mazda's known high servicing costs, they could afford to match!

Just used My Mazda to book my 2nd service late May; on the app, it shows;
1st service (12.5k miles) £170.24
2nd    "       (25k)           £248.88

On the assumption in the 1st 3 years of ownership, you have 2 x£170.24 +
1 x£248.88, that'd total (not allowing for likely inflation hike for 2022 cost), £589.36

I assume prices on App include VAT.

The 3 year Extended Mazda Service Plan bought at dealer cost £679.

Have asked MAZDA HO for rationale as to why there's such a premium :(

I can't agree with your logic.  You are assuming the 3rd year servicing would be the same as the 1st.

Look at it this way

3 year extended service plan   £679       -
(minus Year 1 @ £170.24 ) =  £509.76  - remaining in fund
(minus year 2 @ £248.88 ) =  £259.88  - remaining in fund for the 3rd year servicing
(minus year 3 @ £259.88 ) =  £0 

That makes the year 3 servicing only £11 more that for the year 2 servicing for a car that is gradually getting older and needing more attention.
 

Hardly a disaster considering the projected 12.500 miles

Even if there were 2 of the 2nd services and one 1st; you're still out, as
you calculated.
Some bent logic from Mazda.

As the car's in warranty during this period I'm covered for other eventualities, so
"a car that is gradually getting older and needing more attention" not pertinent.
